Les études cliniques randomisées visant à démontrer l'efficacité d'un médicament dans le maintien de l'abstinence ne concernent souvent que des patients très sélectionnés pour être inclus dans un protocole expérimental et l'abstinence absolue est l'unique critère de jugement. Dans des conditions plus « naturalistes » (étude ouverte descriptive), le suivi et la consommation d'alcool de 215 patients récemment sevrés et inclus dans un projet de traitement par 50 mg par jour de naltrexone a été évalué à JO, J14, J28, J56 et J84 sur les critères suivants : consommation déclarée d'alcool, transaminases et GGT, appétence, « besoin impérieux », fonctionnement global, tolérance et compliance à la naltrexone. Parmi 192 patients retenus pour l'analyse de l'efficacité à J84, 41,1 % des patients se sont déclarés abstinents, 20,3 % contrôlaient leur consommation (consommation d'alcool moins de cinq jours par semaine, maximum quatre verres par jour; trois pour les femmes, maximum quatre jours par semaine) et 38,6 % avaient rechuté. La compliance était bonne dans 76 % des cas. L'analyse de l'évolution de la consommation d'alcool à trois mois retrouve une classe intermédiaire de consommation entre l'abstinence et la rechute : la « consommation contrôlée ». L'éventuelle implication de la naltrexone dans le contrôle de la consommation d'alcool par l'intermédiaire d'une réduction des effets positifs de l'alcoolisation est évoquée. (Résumé d'auteur) ENGLISH : Randomized clinical trials designed to demonstrate the efficacy of a medicinal product in maintenance of abstinence often only concern patients who have been carefully selected for inclusion in an experimental protocol, using absolute abstinence as the sole assessment criterion. Under more "naturalist" conditions (open descriptive study), the follow-up of alcohol consumption of 215 recently weaned patients included in a treatment project with 50 mg per day of naltrexone was evaluated on D0, D14, D28, D56 and D84, on the following criteria: declared alcohol consumption, transaminases and yGT, appetency, "cravings", global functioning, safety and compliance with naltrexone. Among the 192 patients eligible for analysis of efficacy on D84, 41.1 % reported to be abstinent, 20.3 % controlled their consumption (alcohol consumption less than five days per week, maximum four glasses per day; three glasses per day on a maximum of four days per week for women) and 38.6 % had relapsed. Compliance was good in 76 % of cases. Analysis of the course of alcohol consumption at three months revealed an intermediate class of alcohol consumption between abstinence and relapse: "controlled consumption". The possible role of naltrexone in the control of alcohol consumption by reduction of the positive effects of alcohol intoxication is proposed. (Author' s abstract) |
