Nous présentons une étude toxicocinétique chez 4 sujets intoxiqués au méthanol, traités par administration d'éthanol (dose totale allant de 505 g à 682 g) et hémodialyse. Les concentrations maximales de méthanol déterminées à l'admission étaient de 192 à 350 mg/l. Trois des quatre patients ont survécu. Le temps calculé de demi-vie du méthanol, influencé par l'administration d'éthanol et l'hémodialyse, était de 4,29 à 6,54 heures. Chez les patients qui ont survécu, le pH sanguin a été normalisé en 6 à 11 heures et la réserve alcaline en 4 à 12 heures. Le degré de réduction du pH, le trou anionique et le délai avant de commencer le traitement sont des facteurs pronostiques plus importants que la concentration de départ du méthanol. (Résumé d'auteur) ENGLISH: The toxicokinetics of methanol were measured in four male patients poisoned by methanol, who were treated by ethanol (total doses of 505 up to 682 g) and hemodialysis. The concentration of methanol upon admission to the hospital ranged from 192 to 350 mg/L. For three patients the therapy was successful. The calculated biological half-life of methanol, influenced by administration of ethanol and hemodialysis, ranged from 4.29 to 6.54 hours. In the surviving patients, blood pH normalised after 6 - 11 hours and base excess after 4 - 12 hours. It can be concluded that the prognosis is better correlated with the blood pH, the anion gap and the delay before the beginning of therapy, than with the initial methanol concentration.
