Nous présentons le cas de Monsieur W., 56 ans, médecin du travail, souffrant d'alcoolodépendance, comme exemple d'une "thérapie orientée schémas". Le protocole thérapeutique s'inspire des fondements théoriques de la notion de schémas en psychopathologie cognitive et des biais d'attribution qui y sont liés. L'idée est que l'alcoolodépendance est, dans certains cas, une caractéristique discriminante très saillante, que certains patients envisagent comme fédératrice des personnes qui la possèdent. Ainsi, comme Monsieur W., des patients peuvent considérer certaines pensées comme communes à tous les alcoolodépendants et absentes chez les personnes sobres. De cette idée, nous avons envisagé un protocole de restructuration cognitive rapide dans lequel sont comparées les réponses au questionnaire des schémas précoces de Young et Klosko (1993) de Monsieur W., avec les réponses qu'il attribue lui-même à deux célébrités pour lesquelles, implicitement, la consommation d'alcool est une variable discriminante. (Résumé d'auteur) ENGLISH : We present the case of Mr W., 56 years old, an alcohol-dependent occupational health physician as an example of "schema therapy". The treatment protocol is based on the theoretical principle of schemas in cognitive psychopathology and the related attribution biases. Alcohol dependence is considered to be, in some cases, a very prominent discriminant characteristic that some patients consider to be a federating aspect of people presenting this characteristic. Patients like Mr W. can consider some thoughts to be common to all alcohol-dependent subjects and absent in abstinent subjects. On the basis of this idea, we considered a rapid cognitive restructuring protocol in which Mr W.'s responses to the early schema questionnaire of Young and Klosko (1993) were compared to the responses that he personally attributed to two celebrities for whom drinking is implicitly a discriminant variable.
